During perimenopause, a woman’s ovaries begin to produce less estrogen and progesterone and cycle unpredictably; by menopause, estrogen and progesterone levels drop to very low steady levels. Customized Estrogen & Progesterone BHRT


Customized Estrogen & Progesterone Therapy: The foundation of menopause treatment is estrogen therapy – replacing the estrogen that is no longer produced can dramatically improve quality of life. We typically use bioidentical estradiol (chemically identical to the primary estrogen your ovaries made) in either a transdermal form (patch/cream/gel) or occasionally pills or even pellets so no need for taking every day; it can last for 3 to 6 months. Estrogen therapy is highly effective for classic menopause symptoms like hot flashes and night sweats, as well as vaginal dryness and discomfort . If a woman still has her uterus, we always prescribe progesterone alongside the estrogen – specifically bioidentical micronized progesterone – to protect the uterine lining. Estrogen alone can overstimulate the endometrium and increase risk of uterine cancer, but adding progesterone eliminates that risk by keeping the lining thinned and healthy . (Women who have had a hysterectomy typically do not need progesterone.) Progesterone has the added benefit of helping with sleep and mood for some women, and we usually give it as a capsule at night (it can have a calming, sedative effect).

Women often are surprised at the variety of issues that were related to menopause until they experience relief by restoring hormones. These include hot flashes (sudden waves of heat often with sweating) and night sweats that disrupt sleep. Estrogen replacement is the most effective treatment known for these symptoms.

Menopause can cause vaginal dryness, irritation, and pain with intercourse (dyspareunia), as well as urinary issues like urgency or recurrent UTIs. Estrogen, especially if delivered locally (vaginal estrogen), can dramatically improve vaginal tissue health – easing dryness and making sex comfortable again . It also helps maintain bladder and urethral tissue integrity.

One major benefit is bone health: estrogen therapy prevents bone loss and can reduce the risk of osteoporosis and fractures after menopause . There is also evidence that starting estrogen around the time of menopause may have positive effects on heart health and cognitive function – for instance, some data suggests it might lower the risk of heart disease and even dementia if begun in early postmenopause . (These potential benefits are an area of active research and can depend on timing and the individual; we can discuss what current science shows during your visit.) Hormone therapy is not right for everyone – it does carry some risks, which can include blood clots, breast cancer, or stroke in certain populations . However, for a healthy woman in her 40s or 50s with significant menopause symptoms, the benefits often outweigh the risks when therapy is started soon after menopause . We take into account your personal and family medical history (for example, if you have a history of blood clots or an estrogen-sensitive cancer, we may opt for non-hormonal strategies).
